I was once seeing a psychiatrist who insisted that I had to pay for sessions I could not attend due to being out of town covering medical conferences, but was perfectly okay with going away for the month of August. Sure, we all get to have time off, but demanding I pay for sessions that I couldn't attend–for whatever reason, including MY taking time off—was ludicrous to me when at the same time, she was taking an entire month off.
